Students in Transition Achieving
their Goals,
Empowered
& Self-Confident.
Students (14-26) with
disabilities are referred by teachers, parents & service agencies
in Midland, Bay & Saginaw Counties.
Youth gain self-confidence and develop skills that enable them to live
independently when they leave the school environment.

DISCOVER
Taught in a group
setting, this program focuses on helping people with disabilities
identify barriers and personal skills that may contribute to employment
success. Offered in partnership with Michigan Rehabilitation Services.

READY
A one-on-one
pre-employment Readiness and skills development program for people with
disabilities. This program is in collaboration with Michigan
Rehabilitation Services.
Independent
Living
SUPPORT COORDINATION
- Assisting to link and coordinate serivices so people can continue to
live independently in their own homes.
*Now includes supports services designed to aid Veterans
with

HOME
SUPPORT SERVICES - Assist people in hiring, scheduling and coordinating
support to help preserve their independence.
PERSON-CENTERED PLANNING - A guiding principle of all independent
living services, PCP encourages people with disabilities to identify
and reach their goals by helping them develop a life plan.
PASREP - For persons who want to
work, but are concerned with their
Personal Assistance costs, Personal Assistance
Services Reimbursment
for Employment Program may be able to help.
