Individuals with disabilities have the right to make choices about their own lifestyle, the supports they want and those they don’t. A person-centered plan works with you and your support team to help plan for the future. A person-centered plan takes into consideration your dreams, ideas, worries, needs and desires.

A person-centered plan helps you work together with your support team. If you want to build more supports in your life, or simply turn your focus to the future, you can create your own plan and start toward the life you choose!

Independent Facilitators are people trained to help run planning meetings. They are unbiased community members there to keep the focus on your plan for the future.

Why choose an independent facilitator?

The choice to use an independent facilitator is up to each individual. Independent facilitators help explore options and develop plans that focus on your choices. Their role is to ensure the plan is what you want. When an independent facilitator is utilized, the entire support team can become actively involved in planning and brainstorming. This results in a more creative and well-balanced person-centered plan!

We provide personalized support to individuals who need assistance accessing support in their communities.  We work to help meet your goals and create the lifestyle you choose.
